Please read this form carefully and be aware in registering yourself for participation in MIM Tango classes and/or practice you will be acknowledging the risk and releasing all claims which you may have as a result of participating in these events.

I recognize and understand that participation in MIM Tango classes and/or practicas are voluntary, and that I incur risks by participating, including the possibility of death, injury, and/or exacerbation of any existing injury. To the fullest extent permitted by law, I hereby waive any rights I may have to sue MIM Tango with respect to personal injury or property damage suffered by myself as a result of my participation in these events, and hereby release MIM Tango from any liability for such injury or damage. I understand that I am responsible for my actions while I am participating in MIM Tango classes and/or practicas and that I am solely responsible for deciding whether or not I choose to participate or continue to participate in these events.
